Mostly it just started with the 3e dragons - the designers had said that the dragons were under-CR'd by about 3 or 4, to reflect the status that they were "boss monsters". This makes no sense, of course, because sometimes dragons aren't boss monsters and even then, when you're LOOKING for a boss monster, you'll pick someone of a higher CR anyway.
This whole "Actually CR X+3, listed as CR X" was referred to as a hidden "Awesome" tag.

So I'm of a mind to think that its anything with:
Good bab
Decent Magical attacks
Good hp/cr rating.
ahh.. yeah
Immunities.
dr/fuck off
spell resitance: high (if spell resitance matters in your world)
I'll try to find examples but I'm sure most angels/Fiends fall into that.
I'm thiking magical beasts with spells, as well as monsterous humanoids with spells fall into that category alot, but thats not to say those guys inherently do so, in the way that outsiders tend to do.

Nearly anything with spellcasting as a caster at or near its CR is automatically [Awesome], because most spellcasting monsters have better benefits than PC-classed spellcasters. A couatl is CR 10, has 9d8 HD, and casts spells as a 9th level sorcerer (with access to the cleric list). It also gets flight, a poisonous bite, stats better than the average PC, a nasty grapple, ethereal jaunt, and plane shift at will (so it can send you to your death on a successful touch attack if you fail a DC 20 Will save, which is pretty respectable).
It's kind of amazing that a human 14th-level cleric NPC is supposed to be CR 14, but that a trumpet archon (which casts as a 14th level cleric on top of all its other benefits) is also CR 14 despite the fact that it has continuous flight, DR/evil, continuous double-strength magic circle, SR 29, and AoE paralyze on a failed Fortitude save.

I forget which Tome the [Awesome] tag was covered in, but seeing as it's already been described here ("It's stronger than the listed CR would suggest, sometimes intentionally to be a Boss Monster, sometimes through not paying attention/stupidity"), I guess it doesn't matter which.
